- Please use and submit the Basic Safety AHA. Project-specific AHAs will be requested later in the project.
- The Lockout/Tagout (LO/TO) Program is incomplete. Beyond the general information provided, the program must clearly identify the procedures, responsible persons, competent personnel, and any other relevant personnel. Please submit the complete and adequate LO/TO Plan.
- The Certificate of Insurance (COI) must be submitted in order to approve mobilization. Do not forget to include Workers’ Compensation.
- The Safety Data Sheets (SDS) are outdated, with several documents dating back to 2005, 2007, and similar. The SDS must be updated; it is recommended to visit the manufacturer’s website to obtain the most current information.
