- Please review and resubmit your Safety Plan.
- Submit the OSHA 300A for the years 2024, 2023, and 2022.
- Use the provided template as a reference and resubmit the Appointment Letter. You submitted the same list three times (as Key Personnel, as Emergency List, and as Appointment Letter). In addition, please correct the lists to include the names of the individuals. The lists you submitted currently show only the job titles without the corresponding names.
- Submit the Basic Safety AHA. Any other AHA will be rejected. The required template was provided in the mobilization form.
- Submit the COI including General Liability and Workers Compensation. Also, please make sure to include the appropriate description, as you only submitted a vehicle insurance policy.
- Attach the appropriate SDS. You submitted what appears to be your Safety Plan. That Safety Plan may belong under item #1, while the SDS must be correctly submitted under the SDS section.
- You only submitted one worker’s OSHA 10. Will you not have more workers on this project?
- You submitted the CPR certificate for Cristofer Pinzones, who I understand is the PM of the project. Please clarify: will the PM be physically present on site at all times during your activities? The CPR/First Aid requirement must be fulfilled by the supervisor who will be on site, as the purpose is to ensure proper emergency assistance. If the trained individual spends most of the time in the office or rotating between projects, the requirement would not be properly covered.
- In the Fall Protection section, you attached the Mobilization Checklist, which is not correct.
- In the Operator Certifications section, once again, only the certification of the PM was submitted. Please clarify this by email to rq@legocc.com, since as of now only the PM would be authorized to operate the AWP.
