Current Observations (04/07/2026)

  1. The Safety Plan you submitted is more of a checklist. A company Safety Plan should be more detailed, including forms, important data, clear and specific plans such as the Emergency Plan, Fall Protection Plan, Fire Hazard Plan, Inclement Weather Plan, roles and responsibilities, and more. I am not requesting a plan of hundreds of pages, but it must be properly developed. Unfortunately, this plan does not cover the basic requirements to be considered a Safety Plan and it does not even include a name or signature.
  2. Attach the appropriate and company-specific Fall Protection Plan. This observation will be considered addressed according to the Safety Plan you submit, as the submitted document alone is not complete. For example, it does not detail procedures, inspection forms, or clearly assign responsibilities for approvals and sanctions. Even basic sections like LADDER SAFETY do not indicate the need for INSPECTION BEFORE USE, or vaguely state “use proper ladder rating.” What is considered a proper ladder?
  3. The WC expires on May 24, 2026. It is necessary to have a copy of the new WC. Please note that if the WC is not current, project activities will be stopped.
  4. Submit the appropriate SDS. The SDS requested is not a plan — the Chemical Hazard Control Plan should be included in your general Safety Plan (which is currently not included). This section should attach the SDS for the chemical products you use. I have provided an example in the following link: [link]
  5. Confirm via email to rq@legocc.com if there will be only one worker on the project. This is requested because you only submitted one OSHA 10, i.e., only Alexander Wallace, even though he has a 2013 OSHA 10, he would be the only authorized worker on the project.
  6. Attach the appropriate Fall Protection certification. If your team will work at heights, we must verify that your workers have the necessary training.
  7. NOTE: Only Javier Garcia is authorized to use the Scissor Lift on this project. I can see that Javier Garcia is the Supervisor and the only person with Scissor Lift certification. Please confirm that your Supervisor will also be the only approved person to operate the Scissor Lift.
  8. Use the Basic Safety AHA template. In the form you submitted, the document was clearly marked to be downloaded and filled out. You only need to download and complete it correctly. I am attaching a copy of the Basic Safety AHA template. Please complete the first page (top left corner) and page 14 (Competent Person and Signature). It is important to read the document, as by signing and completing it, you accept its content.
