The MOBILIZATION CANNOT BE APPROVED for the following reasons:
A) Recommended improvements to the Safety Plan, focused on regulatory compliance and OSHA standards:
- Formal integration of the Fall Protection Program. Whenever work is planned at heights greater than 6 feet, the Fall Protection Plan must be properly developed, proposing adequate controls, appropriate levels of supervision, and other relevant measures.
- Documented training records and follow-up.
- Emergency response plan including maps and clearly defined roles.
- Documented control of equipment inspections and certifications.
B) Use the Appointment Letter Template provided in the form as a reference. Please note that the appointment letter is not a contact list.
C) Use the Basic Safety AHA included in the form. Specific AHAs will be collected at a later stage.
D) In the Fall Protection Plan, please add:
- The name of the person who created the plan. And Signature
- The credentials of the competent person who developed the plan.
E) Include the appropriate SDS. In the document submitted, you sent the Safety Plan again instead of the SDS. At this stage, you must include the corresponding SDS.
F) Provide the Fall Protection certification of the competent person who will be present at the job site.
G) It is noted that the only person authorized to operate the AWP is Emilio Rivera.

Item E - Our scope is limited to selective demolition only; we do not supply any materials onsite. Can you please clarify the specific SDS we would need to submit for selective demolition?
R.- Regarding the SDS, the answer depends on the following: Will you be using gasoline? Will you be using any type of cleaners? Will you be using any substances other than construction materials? The SDS refers to any chemical that will be used on the project—for example, gasoline to refill a concrete saw, or diesel for the boom lift or heavy machinery. There is almost always something that requires an SDS. You will need to review this detail and let me know.
Item G - Emilio Rivera is the Sunbelt trainer, Erick Vasquez & Ronald Tan are H3FOUR certified AWP operators.
R.- You are correct—the trainer’s name is Emilio Rivera, therefore Erick Vasquez and Ronald Tan are approved for AWP.
NOTE: Since you are performing demolition work, you should also include a Lockout/Tagout (LOTO) Plan in your Safety Plan.
