- A) Your safety plan indicates the use of a “Safety Monitor” as an option. Under LEGO Co. policies, the Safety Monitor is not acceptable. Please modify your safety plan to comply with LEGO Co. policies.
B) You must include a proper incident investigation procedure, and also include the form that will be used to report incidents if they occur. - In the Key Personnel section, you attached the safety plan again, which is not acceptable. Please remember that in your safety plan and Key Personnel section, you should only mention personnel from your own company, not LEGO Construction staff.
- In the Emergency List section, you again attached the safety plan, which is not acceptable. Please remember that in your safety plan and Key Personnel section, you should only list personnel from your own company, not LEGO Construction staff.
- Please use the Appointment Letter template to submit the appropriate information.
- The Fall Protection Plan you submitted is incomplete. It does not indicate procedures, policies, control systems, competent personnel, work stages, risk evaluation, emergency measures, etc. Please submit a complete Fall Protection Plan that includes all necessary sections, including the rescue plan.
- You must submit OSHA 10 certificates for all workers.
- The American Red Cross certificate you submitted is expired. Please submit a valid certificate.
- You have not submitted the Scissor Lift and Boom Lift Operator certificates.
- You must submit the Basic Safety AHA, as this procedure is mandatory. Since your work will involve working at height, you must also submit a separate AHA for working at height.
Dear subcontractor, the information provided through this form is of critical importance to the project. Copying and pasting the same document multiple times or submitting expired documentation is not acceptable. I kindly ask that you take the necessary time to carefully review the form and submit the appropriate information. Your safety plan, in general, lacks many details; however, given the nature of your work, I am focusing my review on the basics plus working at height requirements.
